The Toronto Grace provides medically complex and specialized care and services to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation and Palliative Care.
The care and services provided at the Toronto Grace help facilitate the patient flow through the health care system and ensures recovery for individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities.

At The Salvation Army Toronto Grace Health Centre we are proud to be recognized as a Best Practice Spotlight Organization (BPSO) Designate by the Registered Nurses’ Association of Ontario (RNAO). The achievement of BPSO designate is the development of an evidence-based Best Practice Guidelines (BPG) culture that has required the concerted efforts of all our staff, working at all levels of the organization.
