Complex Continuing Care

The Salvation Army Toronto Grace Health Centre (TGHC) is committed to providing compassionate, patient-focused care and services to all individuals who require our help.  Our values of dignity and respect have equipped us to be leaders in Complex Continuing Care.

Care and services are provided to patients who have multiple complex care needs requiring ongoing and frequent medical/nursing assessment and intervention; compromised respiratory systems that require ongoing specialized intervention, pressure ulcer and wound management, and care needs that exceed the capability of community care providers. The goal is to enhance abilities and improve function. Complex Continuing Care is an interim resource, not a destination. The program ensures that patients are encouraged to participate in their own care and to enjoy choice.

Accommodation and associated costs: (Please note that all associated costs are subject to change).

Patients are charged a daily Ministry of Health and Long-Term Care mandated co-payment fee. Subsidies are available to qualified applicants.

  • Co-payment maximum $1,891.31/month ($62.18/day) based on patient’s income.
  • Preferred accommodation: private $85.00/day; semi-private $50.00/day.
  • Television: $75.00/month; $10.25/day; Telephone: $35.00/month; $3.08/day.
  • Parking: reduced rates for hospital visitors, and parking passes available.

Off-site treatments (laboratory, diagnostic, surgical) and appointments (fracture clinic, specialists) may not be available at Toronto Grace. When necessary, the care team will arrange your treatment at another health care facility.

Patients who become acutely ill and require emergency care will be sent to the closest health care facility.  A nurse will place a 911 call for an ambulance transfer and provide treatment as ordered by the physician until the ambulance arrives.

What should I bring for my inpatient stay?

Ontario Health Card.

Health insurance information.

Comfortable clothing; running/supportive shoes; personal items (pajamas, bathrobe, slippers – non-skid); toiletries (toothbrush, toothpaste, shaving supplies; shampoo, bath soap and dressing aids where applicable).

At TGHC we cannot accept responsibility for lost, stolen or damaged items. We advise that you do not bring large amounts of cash, jewelry or other valuables.
