Interprofessional Health Care Team


The Toronto Grace Health Centre TGHC) has an experienced Interprofessional Health Care Team which consists of practitioners from different professions, and who share three distinct patient populations and common patient care goals.

The teams have an established means of communication amongst themselves and with patients and families to ensure that all aspects of the patients’ health care needs are integrated and addressed.


Our team is comprised of:

  • Chaplains;
  • Chiropodists;
  • Clinical Dietitian;
  • Communications Disorders Assistant;
  • Nursing;
  • Occupational Therapists;
  • Patient Care Coordinators;
  • Pharmacists;
  • Pharmacy Technicians;
  • Physicians;
  • Physiotherapists;
  • Recreation Activity Assistant;
  • Recreation Therapist;
  • Rehabilitation Assistants;
  • Registered Respiratory Therapists;
  • Speech Language Pathologist;
  • Social Workers;
  • Unit Clerks, and
  • Volunteers

The focus of the teams is on the needs of the patient. Patient care goals determine the composition of the team that will be responsible for a particular patient problem/issue. The patient and family are empowered to become partners in their care.
