Interprofessional Practice

An integrated approach to care is vital in addressing patient needs holistically; which is a core component of an interprofessional practice.  In an interprofessional team approach, all health care professionals are encouraged to work collaboratively in meeting patients’ needs and concerns.  It encourages a collaborative approach to problem-solving and decision-making with a patient-focused perspective.  To promote interprofessional practice at the Toronto Grace, an Interprofessional Practice Council (IPPC) was developed.

The Interprofessional Practice Council (IPPC) enables representatives from various health care professions (such as; Clinical Dietitian, Pharmacy, Nursing, Occupational Therapy, Physiotherapy, Social Work, Speech-Language Pathology, Spiritual Care, Recreation Therapist, Patient Care Manager and Medicine) to facilitate a discussion surrounding current practices, policies and procedures, and existing interprofessional issues and concerns.  As a result, the council formulates quality improvement strategies to further enhance interprofessional practice.

Interprofessional practice is one of the core components of the New Model of Care which strives for a collaborative approach to patient care.  Therefore, we value and support interprofessional best practices through:

  • Representation in academic institutions;
  • Preceptorship/Mentorship of various clinical students;
  • Active involvement in clinical presentation, rounds, and conferences; and
  • Participation in research, external/internal education events, and journal publications.

Teamwork is the cornerstone of our interprofessional practice which is another perspective that the Toronto Grace values.  With interprofessional practice, we are able to provide a patient-focused and holistic approach to care.
