An integrated approach to care is vital in addressing patient needs holistically; which is a core component of an interprofessional practice. In an interprofessional team approach, all health care professionals are encouraged to work collaboratively in meeting patients’ needs and concerns. It encourages a collaborative approach to problem-solving and decision-making with a patient-focused perspective. To promote interprofessional practice at the Grace, an Interprofessional Practice Council (IPPC) was developed.
The Interprofessional Practice Council (IPPC) enables representatives from various health care professions (such as; Clinical Dietitian, Pharmacy, Nursing, Occupational Therapy, Physiotherapy, Social Work, Speech-Language Pathology, Spiritual Care, Recreation Therapist, Patient Care Coordinator and Medicine) to facilitate a discussion around current practices, policies and procedures, and existing interprofessional issues and concerns. As a result, the council formulates quality improvement strategies to further enhance interprofessional practice. The Council is co-chaired by the Professional Practice Leader and the Nursing Practice Leader both of whom are members of the Professional Practice Network of Ontario (PPNO) to which they can incorporate interprofessional practice approaches from other facilities across Ontario to the Grace.
Interprofessional practice is one of the core components of the New Model of Care which strives for a collaborative approach to patient care. Therefore, we value and support interprofessional best practices through:
- Representation in academic institutions;
- Preceptorship/Mentorship of various clinical students;
- Active involvement in clinical presentation, rounds, and conferences; and
- Participation in research, external/internal education events, and journal publications.
Teamwork is the cornerstone of our interprofessional practice which is another perspective that the Grace values. With interprofessional practice we are able to provide a patient-focused and holistic approach to care.
