Maria, a 24 year-old patient, received a bi-frontal craniotomy with a trach insertion at Sunnybrook Health Sciences Centre.  She was admitted to the The Salvation Army Toronto Grace Health Centre for Complex Continuing Care (CCC).  She was medically stabilized and her caloric intake was assessed.  Gradually she received respiratory therapy and her trach was removed.  Maria also received speech therapy with our speech-language pathologist.  She eventually transitioned from g-tube feeds to eating by mouth.  Maria began rehabilitation with a physiotherapist and occupational therapist to increase her strength; in time she was able to return home on weekends.  The CCC unit along with the Community Care Access Centre assisted the family with operation of in-home equipment (bed and lift).  Maria is now at home.
