Our patient-centred care focuses on the physical, mental and spiritual needs of a complex patient population in the Palliative, Complex Continuing Care and Slow-Paced Rehabilitation programs.
Significant change for the Ontario Health Care systems is underway, resulting in increased insight into treatment opportunities within the Complex Continuing Care sector. Thus, innovative modalities in care have been implemented at The Grace. Toronto Grace Health Centre is committed to our partnership in the Toronto Central Local Health Integration Network and to being a transforming influence in the provision of care and services to meet the needs of the community we serve.
Our focus is to integrate our services in Toronto; finding solutions and championing the needs of our patients. The community benefits significantly from having access to the type of care we provide. Caring for those entrusted to us is regarded as a sacred trust.
