Complex Systems Support Program
The Complex Systems Support
Program (CSSP) provides patient centred care designed to improve the quality
of life of individuals who have chronic diseases and/or complex medical conditions including medically complex geriatrics.
Patients receive skilled clinical treatment and many participate in slow-paced,
long duration rehabilitation with the goal of successful discharge to the
community or to an active rehabilitation program.
Admission Criteria

Adults, 18 years and older
Progressive conditions requiring ongoing medical care and 24/7 nursing care.
Patients
requiring slow-paced
rehabilition need to have goals for at minimum two therapeutic interventions
(e.g. nursing for wound care, physiotherapy, speech language pathology etc.)
Patents
have an anticipated ability
to learn and participate in care.
Complex medical conditions may include:

cardio-respiratory diseases,
endocrine dysfunction,
mental health impairment,
diabetes,
cognitive impairment
deconditioning as a result of prolonged hospitalization
transplants
renal disease including patients on heamodialysis (dialysis done at acute site)

Admission applications are accepted from acute care, rehabilitation, long-term care, community programs and primary care practitioners/teams.
Patients care needs may include non-ventilated tracheostomy care, enteral feeding, ostomy care, wound management and functional restoration.
