Adults, 18 years and older
Patient is medically stable
Patients with significant disability or altered functional status following a stroke or major neurological event or those living with debilitating neuromuscular diseases.
Complex neuromuscular conditions include:

Stroke,
Multiple Sclerosis,
Gullian-Barre syndrome,
neurodegenerative and neuromuscular disorders such as
|
|
|
Moyamoya disease,
LaFora Body Syndrome
Creutzfeldt-Jacob disease.

|
Patients are accepted into slow- paced rehabilitation program (SPR) as per the following criteria:

|
Patient requires 2 or more interdisciplinary interventions (e.g. physiotherapy, wound management, SLP)
Expected discharge destination is return to pre-acute living arrangement
Patients have an anticipated ability to learn and to participate in care
Patient is motivated to attend therapy and contribute to their rehabilitation
