Adults, 18 years and older
Patient is medically stable
Patients with significant disability or altered functional status following a stroke or major neurological event or those living with debilitating neuromuscular diseases.
Complex neuromuscular conditions include:

multiple sclerosis
Gullian-Barre syndrome
neurodegenerative and neuromuscular disorders such as:

Moyamoya disease
LaFora Body Syndrome
Creutzfeldt-Jacob disease

Patients are accepted into slow- paced rehabilitation program (SPR) as per the following criteria:

Patient requires two or more interdisciplinary interventions (e.g. physiotherapy, wound management, SLP)
Expected discharge destination is to return to pre-acute living arrangement
Patients have an anticipated ability to learn and to participate in care
Patient is motivated to attend therapy and contribute to his or her rehabilitation
