Patients
Transitional Care Unit (5th Floor)
Provides ongoing clinical management aimed at improving/restoring the functional abilities and quality of life of individuals who have multiple physical, cognitive and/or social issues that impede their presenting level of independence, chronic diseases, fractures, and/or require long-term palliative care. This is a 32-bed unit consisting of four-bed, two-bed and one-bed rooms with bathrooms in each room and therapy gyms on the unit.
Admission Criteria

Adults, 18 years and older
Patient is medically stable
Conditions may include:
|

Parkinson's
Long-term palliation (6-12 months or longer)
mental health impairment
fractures - pre and post operative
soft tissue injuries
diabetes
deconditioning as a result of prolonged hospitalization

Patients are accepted into slow-paced rehabilitation program (SPR) as per the following criteria:

|
Patient requires two or more interdisciplinary interventions (e.g. physiotherapy, wound management, SLP)
Expected discharge destination is to return to pre-acute living arrangement
Patients have an anticipated ability to learn and to participate in care
Patient is motivated to attend therapy and contribute to his or her rehabilitation
