Programs & Services

Palliative Care provides expert holistic and respite care for patients, families and friends through inpatient and community outreach programs. These programs provide ongoing medical management, skilled nursing and a range of interprofessional therapeutic services using a supportive symptom management approach. Palliative services are offered for people who are in the last stages of their life as a result of cancer, end-stage disease (such as ALS, HIV/AIDS) and end-stage organ failure. Patients are recognized as having unique needs and values and have the opportunity to fully participate in decisions affecting their life and health. Support of family, as identified by the patient, is essential to the care provided on the palliative care unit.
The Grace's Complex Continuing Care programs provide person-centered care for adults with functional impairments and multiple co-morbidities in an abilities-focused model. These programs provide ongoing medical management, skilled nursing and a range of interprofessional therapeutic services. They offer individual assessments of patients and a restorative, preventive approach so that the patient can return home or receive an alternate level of care in the community. We provide a caring, respectful environment that actively engages the patient, family and community in the day-to-day experience.
