Patient & Visitor Rights and Responsibilities
Every Patient has the Right to:
Be
treated with respect and dignity at all times
Express
his/her personal opinions, feelings and values without fear of
recrimination
Be
free from abuse of any sort: psychological, physical or verbal
Be
informed regarding his/her medical condition and care plan
Know
the members of his/her health care team
Participate
in care planning with the health care team, including discharge planning
Ask
questions about his/her care
Refuse
consent to treatment, including medications, once the rationale for the
treatment and consequences of refusal have been fully explained by members
of the care team
Be
supported in achieving maximum level of independence
Pursue
social, cultural, recreational and religious interests
Have
family members participate in caregiving and stay with the patient as they
wish and as appropriate to the patients health status
Have
confidential management of all personal, medical and financial
information, in accordance with provincial legislation
Access
his/her chart, in accordance with hospital policy regarding access to
information
Live
in a safe and clean environment
Retain
personal items such as clothing, pictures and privately owned equipment,
in keeping with the hospitals safety requirements
Access privately funded care to supplement
care provided at the Grace, in accordance with the External Care Provider
Policy
Every Patient has the Responsibility to:
Treat
other patients, staff, volunteers and visitors with dignity and respect
Inform
the health care team of his/her previous health history including
prescribed medications, vitamins, supplements and over the counter
medications taken
Follow
infection control practices to support a safe environment, including
washing his/her hands often and when moving from his/her room to other
areas of the health centre
Ask
his/her visitors to use the hand sanitizers as they enter and leave the
patients room and the health centre
Ask
for help getting up and moving about, if he/she has been instructed not to
do this on his/her own, in order to prevent falls
Inform
staff if he/she has any concerns about safety
Inform
staff if he/she is leaving the unit
Wear
patient identification bracelet
Use
hospital equipment, such as a wheelchair, in a safe manner, as instructed
by health centre staff
