Our President & CEO, Dr. Jake Tran and Danielle Kilby-Lechman, Director of Integrated Transitional Care, were invited as expert speakers to share insights on “Scaling Remote Care Monitoring for System Transformation.”

Their presentation highlighted how Toronto Grace is advancing virtual care and helping seniors remain safely at home through innovative Remote Care Monitoring programs.

Toronto Grace Health Centre was pleased to welcome back the Your Health Space (YHS) program.

In partnership with the Canadian Mental Health Association Ontario, YHS delivered 10-minute Wellness Moments focused on Managing Stress and Mindfulness. Teams across our units participated in engaging huddles featuring practical strategies, helpful resources, and wellness activities.

Thank you to everyone who took part in supporting a healthier, more resilient workplace.

Toronto Grace Health Centre is proud to support the recent announcement at Isabel and Arthur Meighen Manor health centre, where the Ontario government launched a three-year, $9-million pilot project focused on advancing emotion-focused care in long-term care homes.

Our CEO, Dr. Jake Tran, attended the event in support of this important initiative, which aims to create more homelike environments and better respond to the emotional and social needs of residents — particularly those living with dementia.

This pilot represents a meaningful step toward transforming long-term care from institutional routines to individualized, relationship-centred care.
We look forward to seeing the meaningful impact this initiative will have across Ontario.
