The Salvation Army Toronto Grace Health Centre (TGHC) health care practices are evidenced-based and utilize an interprofessional team-based approach, with patients and their families acting as drivers of their own care.  The patients are assessed by each member of the interprofessional health care team.  Together, this group creates a plan of care with the patient, and their family/caregiver.

Treatment and care is provided based on Resident Assessment Instrument — Minimum Data Set (RAI-MDS) data and outcome scales, and evidence-based best practices.  A primary nurse is assigned to each patient, and is responsible for providing safe and quality care while collaborating with family members and interprofessional health care team in identifying goals and strategies to achieve the goals identified in the patient’s care plan. Family members/caregivers are considered to be a vital support to the patient and are encouraged to participate and work with the team in the care planning process.  The primary nurse and patient care manager work together to help resolve any concerns that are raised by the patient or family member/caregiver.  They will refer these issues to an appropriate team member or to the entire team depending on the nature of the concern.

The teams have an established means of communication amongst themselves and with patients and families to ensure that all aspects of the patients’ health care needs are integrated and addressed.
