The Salvation Army Toronto Grace Health Centre (TGHC) is committed to providing compassionate, patient-focused care and services to all individuals. The Outpatient Clinical Program aims to provide specialized care services to ambulatory patients in the outpatient setting.
Clinical services are provided to ambulatory patients who have care needs requiring specialist consultation and ongoing care in the community setting. We are passionate about delivering high quality care services and diagnostics for our patients. We welcome new referrals from the surrounding communities and our goal is to create smooth transitions for our patients to help them thrive in their communities. Now available on OCEAN e-referral. The Outpatient Clinical Program is comprised of a series of specialty clinics located on the main floor of Toronto Grace Health Centre (650 Church Street).

How do I get an appointment at the clinic?
Our specialists require a referral from another physician. Speak to your physician or nurse practitioner if you need a referral to one of our specialists. They can fax a referral to 647-689-3554.
What should I bring for my outpatient clinic visit?
- Ontario Health Card.
- Health insurance information.
- Comfortable clothing for assessment: running/supportive shoes & comfortable clothing
Where are we located?
The clinics are located on the main floor at the Toronto Grace Health Centre, at 650 Church Street (corner of Bloor and Church Streets). **Upon arrival all patients with appointments must check-in using the CHIME Kiosks located at the front entrance. Additional support or questions can be directed to outpatient clinic staff office on the left side of the entrance.
Parking is available onsite or Green P lot located on Charles street

Your referral may not have been received by our clinic. Speak to your own physician or nurse practitioner to follow up. If your physician has sent us a new referral already and you’d like to book a time for a new consult, you can email outpatientclinic@torontograce.org
