Outpatient Geriatric Medicine Clinic
We are pleased to welcome Dr. Arielle Berger, who will be providing general Outpatient Geriatric Medicine consultation services.
Dr. Berger is a geriatrician and assistant professor in the Dept. of Medicine at the University of Toronto.
The clinic will see patients for:
- Cognitive Impairment
- Polypharmacy
- Falls and Functional Decline
- Multi-complexity
The Outpatient Geriatric clinic is located on the main floor at the Toronto Grace Health Centre, 650 Church Street (corner of Bloor and Church by the Bloor/Yonge subway station).
Referral Instructions
Eligibility Criteria:
- Connected to a primary care physician/NP
- Not seen by outpatient geriatric medicine in the past 2 years
