Outpatient Geriatric Medicine Clinic

We are pleased to announce the expanding Geriatric Medicine program at Toronto Grace.
Dr. Arielle Berger and Dr. Marly Isen provide geriatric medicine consultation services to inpatients of the Toronto Grace hospital as well as outpatient comprehensive geriatric assessments in clinic.

The Outpatient Geriatric clinic is located on the main floor at the Toronto Grace Health Centre, 650 Church Street (corner of Bloor and Church by the Bloor/Yonge subway station).

Both in-person and virtual appointments are available. No catchment area restrictions.

Our clinic is accepting referrals for evaluation of cognitive impairment, falls, frailty, functional decline, polypharmacy, multi-morbidity/multi-complexity.
