The Salvation Army Toronto Grace Health Centre’s vision is to provide exceptional and compassionate care for all.

We are here to support you through your experience at Toronto Grace and assist you with questions or concerns.

Your Toronto Grace health care team is committed to providing you with the best patient-centred health care possible. They will assist you in understanding your medical condition, explain your options and help you to make decisions about your care. They will work with you to develop and implement your care plan. Should you have any questions at any time, your health care team is available to talk with you or your family/caregiver. 

We welcome any suggestions you have about the way your care is provided, and we want you to know that we take both your health care and comments seriously. Our goal is to meet your expectations.

If you have a concern, a suggestion or a compliment, we would like to hear your feedback. If you have a concern, you can always speak to your care providers or team directly or the Patient Care Manager on the unit. If you don’t feel satisfied after this, please contact us. 

When to call:

  • When you want to share your ideas or suggestions; or
  • When you have concerns about services or interactions with Toronto Grace staff or physicians.

Toronto Grace employees, physicians, and management want to be engaged with our patients and their families in resolving their concerns in a comprehensive, transparent and objective way.

The issues you bring to us will be documented and shared with the right people within the organization, who are committed to improving safety and quality of care at Toronto Grace.

  • by mail: 650 Church Street, Toronto, ON M4Y 2G5 Attention: Patient Experience; or
  • by depositing your feedback in the “Suggestion Box” on the main floor to the right of the elevators and on each floor by the patient care unit stations.

We ensure that all concerns are taken seriously and that everyone is treated with courtesy and respect. Please check bulletin boards for responses to suggestions.
