Community Care Program
The Remote Care Monitoring (RCM) Community Care Program is offering warm conversations (via telephone or video meeting) with RCM patients to help ease the burden of stress associated with being isolated and/or dealing with chronic health concerns. Program offerings include: telephone and video conversations with trained spiritual care staff that will connect with patients, families and caregivers on an ongoing basis.

Active Together T.I.M.E.™ at Home Program
The Remote Care Monitoring (RCM) – Active Together T.I.M.E.™ at Home Program aims to provide physical and social wellness virtually for RCM clients with mobility and/or balance challenges. Through collaboration with Toronto Rehab Institute, RCM – Active Together will be hosting the TIME™ at Home program as part of our virtual programming. TIME™ at Home classes are pre-recorded functional exercises that are evidence-based and designed by physiotherapists. Each class will be led by TIME™- trained volunteers with dedicated time for socialization. Although functional exercises will be performed during TIME™ at Home classes, it is not a progressive one-on-one rehabilitation therapy program.

The Remote Care Monitoring (RCM) – YouTube Channel serves as a central hub where you can access a diverse range of videos designed to enhance your well-being and support your health journey. This platform aims to share engaging and informative wellness-related content tailored for the RCM population. The content of this channel can include topics on functional exercises/physical activity, cognition, health-related tips, etc. We’re passionate about delivering content that matters to you and are always open to new ideas. If you have suggestions for specific wellness topics or types of videos you’d like to see, please don’t hesitate to reach out.
