The Salvation Army Toronto Grace Health Centre (TGHC) is grateful to the Rotary Club of Toronto Charitable Foundation for their gift of $50,000 over two years for the Remote Care Monitoring program. The Remote Care Monitoring program is designed to provide in-home support and care as an alternative to admission to an in-hospital setting for eligible patients, including those requiring acute care, complex continuing care, and rehabilitative care. It uses technological advances to accelerate and expand the ability to discharge inpatients to in-home settings to improve the ability for patients to age in the comfort of their homes. The Rotary Club of Toronto Charitable Foundation’s donation will support this program in providing the equipment and technology needed to those who are otherwise unable to afford it so that their health and wellbeing is carefully monitored and remotely delivered by the appropriate caregivers.
