Welcome to Toronto Grace

The Toronto Grace Health Centre (TGHC) is a 119-bed facility located in the heart of downtown Toronto.

The TGHC provides medically complex, and specialized care and services to those individuals who require Complex Continuing Care, Post Acute Care Rehabilitation and Palliative Care.

The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities.

The Toronto Grace Health Centre patients and staff are temporarily located at UHN Hillcrest site, 47 Austin Terrace, Toronto, ON M5R 1Y8 (off Bathurst St. south of St. Clair Ave.W. & north of Davenport).

Patients, visitors and employees are entitled to be protected from the hazards of second hand smoke. Smoking is prohibited in any enclosed public place or enclosed workplace, in accordance with section 9(1) of the Smoke Free Ontario Act. Smoking is permitted in a designated area in the parking lot.
